Risk Management Guidelines

Louisiana Soccer Association requires that all adult volunteers/staff members complete an online Risk Management disclosure form.  
 

Volunteer/Staff Responsibilities

Every adult volunteer/staff member must complete a disclosure form every year. Click the link below to complete and submit your Disclosure Form for Certification now.

Only the LSA Risk Management committee will be able to view the completed disclosure form.

 

Important note: coach passes will not be distributed until the disclosure form has been submitted and a completed background check has been approved and recorded by the LSA office.

 

Club Responsibilities

The Club Registrar will only be responsible for matching up the disclosures with the appropriate volunteers/staff members in their on club (the club administrator will only have access to the name and address entered on the disclosure, but will not have access to any sensitive information entered on the disclosure form itself).  Refer to the KidSafeOneTM Risk Management section of the LeagueOne User Guide for detailed instructions on matching disclosures to the volunteers/staff in your club.

 

Once the volunteer/staff member has a current disclosure attached to his/her record, the state office can run the background check and then approve the volunteer/staff member.
