Read this before you continue to the online disclosure form.

1.  You should only fill out ONE disclosure per soccer year* no matter how many "soccer jobs" you perform.
      This disclosure will cover EVERY job you may have under the LSA, including COACH, ADMINISTRATOR, REFEREE, VOLUNTEER, ETC.  Many supporters of soccer wear many hats, thanks for all you do but PLEASE only one disclosure per year.

2.  Use your COMPLETE LEGAL name as it appears on your birth certificate -- including full middle name (if no middle name enter NMN) - -  no nicknames please.

3.  Remember to keep a copy of your confirmation number.  If your club registrar is having trouble finding your disclosure, they can easily find it with the confirmation number.

*soccer year runs 8/1 through 7/31.
