UNIFORM PICKUP AND ODP FORM DROP OFF
We still have
uniforms in the office that need to be picked up and forms that need to be
dropped off.
Forms -
You can mail your ORIGINAL forms to the office (475 Gardere Lane, Baton
Rouge, LA 70820) If you just owe me a insurance card, I will be
happy to take that by fax or email.
Uniforms
- If you are in Baton Rouge you can come by any time to get your uniform.
If you cannot come by we will bring your uniform to South Texas for 96 - 99
or to the next training session in Lafayette for 93 - 95.
LSA Office
Hours are - 9 am to 5 pm Monday through Thursday and 9 am to 12 pm on

Need to
exchange something - email me and we will get that exchange worked out.
